Well, yes and no.  Modern medicine does reach the third world too, only later. Understanding the cellular mechanisms may very well also contibute in finding a cure to AIDS and other diseases that plague the third world countries.

See, here's where I disagree with you. There aren't enough doctors in Africa or Asia, and I guarantee you that your average African, Indian, Burmese and maybe even Chinese doesn't have access to health care like we do. They're probably lucky to make it to a clinic once a year, let alone a thorough medical checkup by a doctor.

Yes, modern medicine will probably reach the third world too, but not in our life times and not for the average person.

You're right, of course, the state of medical provision in the 3rd world is parlous and wrong. Might it be better if instead of doing further research we provided free medical care / training to poor countries? Arguably. Will that ever happen? No. Research has to pay for itself. Helping poor people, sadly, never pays for itself. As far as providing help to these people goes, the big advances will be economic rather than scientific. If a few more people bought free trade coffee instead of Nescafe it'd help a lot of people.
