Is your BattlEye up to date? Are you running everything the server is? All the same version?

You have CO on your computer correct? What I would do is delete all existing files/folders and upload (via FTP) my files from Arma 2 OA CO.

Are you installing as administrator?

Try loading a different map, make sure you have the name spelled correctly in your server.cfg .

You might look at the .rpt file to see if it gives you any clues as to why it happened.

Not any different from non-steam. Just copy your files over as you would normally via FTP. Yes, you would need to install the game in order for this to be possible.

Indeed, use TaskManager and close out anything running in background. WSearch is usually on that list causing lag.
