creating Arma 2 Combined Operations on Steam from Arma 2 Complete collection

Apologies if this has been asked before but the guidance which turns up on a Google Search of how to install Invasion 44 is incomprehensible!

I have on Steam Arma 2, Operation Arrowhead, British forces and whatever the fourth one in the pack is - how do I create Combined Operations??
