ACE Healing Problem?

I have had this issue for a few weeks so I am hoping someone can point me in the right direction or help me out. Basically, when someone gets shot and you go to revive them you can't because they fall through the floor. If you know where they fell through the floor at you can revive them but once they wake up and such they are midway in the ground with out any possible solution of getting them out. Any help is appreciated .
