i had encountered the issue before too, sometimes the server your playing can get overstressed and wont really know how to respond. say your on a full server and someone likes to troll by spawning a bunch of cars and leaving them everywhere. although im not to sure if im right, but would be something of a bug, the game tends to be buggy on modded servers

from what ive noticed, the more you get kicked from official servers the longer you get banned. the more of a bad sport you are the more likely you are to get kicked out of a game. i dont think its the devs' fault for getting yourself banned.

Some mods after updates tend to break, your best bet is to uninstall the mod from the workshop. (until the mod gets updated it will remain broken).

I don't play on controller so i dont know how on controller, however M&K. If you just drag and drop the same ammo type of a half emptied mag they will refill.
