How to make add a function or anything to the Custom Controls

Trying to add a function to happen when the Custom Control "Use Action 5", or any of them for that matter, is used... Tried searching for it but I can only find people that have it done with no explanation.. First time posting here so please yell at me about tags or anything else I did wrong...
